The poem is signed E.H.C., Rickford's Hill, Aylesbury - the initials stand Ernst Hartley Coleridge, grandson of Samuel Taylor Coleridge. The latin motto had a special significance for the Coleridge family, and was the motto of his cousing John Duke Coleridge, whose letters Ernst Hartley edited - see p. 282 in Life and Correspondence of John Duke Coleridge, 2: Lord Chief Justice of England.
