An artists' book in the form of a questionnaire, which was probably issued inconjunction with the artists' environment / happening of the same name as part of the Ordeals exhibition at Judson Church that same month. Ordeals had been put together to coincide with the International Congress on Religion, Architecture, and Visual Arts. As Hendricks relates, it was "contrived as a fantastic evocation of many of the real-life horrific and humiliating situations visited upon persons and populations all over the world by individauls and groups vested with authority and power."
In the Garden Room, participants walked by a man with a clerical outfit and a meat cleaver, and were given this 10 question exam.
Rare. OCLC locates no holdings, and this is the first example we've seen.
